The All India Institute of Speech and Hearing (AIISH) here will continue to issue hearing disability certificates in prescribed formats following a direction from the office of the Director General of Health Services, Union Ministry of Health and Family Welfare.
Individuals eligible for the certificates have to submit two passport-size photographs and address proof along with the application form. Forms are available at AIISH, a release issued by AIISH Director S.R. Savitri said.
One of the following original documents can be produced as proof of address: ration card, voter identification card, driving licence, bank passbook, passport, a utility bill (telephone, electricity, water) indicating the address; or certificate of residence issued by panchayat bodies, municipalities or any gazetted officer.
In case of an inmate of a residential institute for persons with disabilities, a certificate of residence from the head of the institute can be produced.
